Debt large business cover sheet

Large businesses and their authorised representatives should use the Debt large business cover sheet template (PDF 68KB) to write to us about account-related matters.

You can use this cover sheet when sending these queries to us by fax or mail. Account-related queries may include, but are not limited to:

  • statement of account
  • account transfer
  • payment arrangement
  • advice of payment
  • remission of general interest charge
  • re-issue of activity statements
  • deferral of lodgment of income tax returns or business activity statements
  • pay as you go (PAYG) withholding information
  • responses to our requests for information from you.

A large business is defined as an economic group or single entity with an annual turnover greater than $250 million. Large withholders (businesses that withheld more than $1 million in a previous income year, or are part of a company group that withheld more than $1 million in a previous income year) also fall within the large business market.

How to use the cover sheet

  1. Open and complete the cover sheet online.
  2. Print the completed document on blank paper only. Using letterhead may cause problems when we scan your documents.
  3. Include as your fax header or cover sheet for correspondence sent by mail. Correspondence can be

  4. Keep a copy of the document and correspondence for your own records.

Attention icon

You must complete a separate cover sheet for each unrelated taxpayer request.

Do not send correspondence for multiple unrelated taxpayers in one fax transmission or by mail using the one cover sheet.

Where correspondence concerns a group of related taxpayers, complete one cover sheet providing details of only the principal or holding taxpayer.
