Who should complete this form
You should complete this form if you are an:
employer, with employees who are eligible to choose a super fund, or
an employee, who is eligible to choose a super fund.
Members of a defined benefit superannuation fund who are eligible and decide to choose a new superannuation fund should also complete this form.
