When we collect a superannuation guarantee entitlement or hold a super co-contribution entitlement for you, we seek to pay it to a super fund account or retirement savings account (RSA) in your name. You complete this form to nominate a super fund or RSA to receive the super entitlements we hold for you.

From 1 July 2013, this form also covers unclaimed super.
If you lodge this form after 1 July 2013 and we hold any unclaimed super for you, we will roll this over to your nominated account from 1 January 2014. If you want to have your unclaimed super rolled over before then, you can use SuperSeeker to make your nomination.

This service provides you with details of all your accounts held by super funds, including the account number and fund contact details, as well as any accounts we hold on your behalf. You can also consolidate your super accounts.
How to obtain this form

To obtain a printed copy of the form:
- use our automated self-help publications ordering service at any time. You need to know the full title of the publication to use this service
- phone our Publications Distribution Service on 1300 720 092. You can speak to an operator between 8.00am and 6.00pm Monday to Friday. Before you phone, check whether there are other publications you may need - this will save you time and help us. For each publication you order, you need the full title
- visit one of our shopfronts between 8.30am and 4.45pm weekdays (except public holidays).

Before providing the details of the superannuation fund or RSA you want to nominate, check that both the following apply to the fund/RSA:
- it is a complying superannuation fund
- it will accept superannuation payments from the ATO.

To check the complying status of your fund/RSA, use the Super Fund Lookup service at ABN Lookup - home.

Contact your fund/RSA to obtain both:
- their Australian business number (ABN)
- the member account number the superannuation payments should be sent to.
We recommend you ensure your fund/RSA has the same name and contact details for you as we have. Any discrepancies in these details could result in delays in processing your superannuation payments.
