Choosing a super fund - How to complete your Standard choice form

The Standard choice form sets out employee options for choosing a superannuation fund.

The Standard choice form (NAT 13080, PDF, 274KB) is available in a downloadable format that can be completed electronically. You can not save data typed into this form. Please print your completed form if you would like a copy for your records.

Who should complete this form

You should complete this form if you are an:

• employer, with employees who are eligible to choose a super fund, or
• an employee, who is eligible to choose a super fund.

Members of a defined benefit superannuation fund who are eligible and decide to choose a new superannuation fund should also complete this form.

If you cannot download the form you can:

• order a copy online, or
• order a paper copy by phoning our publications ordering service on 1300 720 092.
