Who should complete this application?

You should complete this application if you want to request a payment of unclaimed superannuation money held by us in your name. Your unclaimed superannuation money may have been paid to the ATO for one of the following reasons:

  • you were aged 65 years or older
  • you held a temporary visa that has ceased to be in effect (has expired or been cancelled) for at least six months and it has been at least six months since you left Australia
  • you were entitled to a superannuation amount that was paid to the ATO as a non-member spouse account. Such a superannuation account would have been created in your name due to a financial agreement or court order under the Family Law Act 1975 where the superannuation fund was required to split a superannuation amount originally held in your former spouse's or de facto partner's name.

When completing this application you must:

  • provide certified documents to prove your claim
