Supporting document requirements for private rulings

This page lists the documents and information we usually need to process private rulings about death benefit termination payments.

Send the following documents and information with your private ruling application form.

Death benefit termination payments

  • The date of death of the deceased
  • The date the payment was made
  • The source of the payment
  • Was the payment made directly to you or the estate of the deceased?
  • Copies of any payment summaries
  • A copy of the deceased's last will and testament
  • Details of the relationship between you and the deceased
  • Details of where you resided prior to, and at, the date of the deceased's death
  • Details of financial support received by you from the deceased prior to, or at, the time of their death
  • Details of financial support received by the deceased from you prior to, or at, the time of their death
  • Details of domestic support and personal care you and the deceased provided each other prior to their death
  • Any available documentary evidence that would support the statements made in your private ruling application
