How to complete the Voluntary release authority for excess contributions tax and statement

Download a PDF of Completing the voluntary release authority and statement (NAT 71777, PDF, 386KB) which includes a sample of the form.

About this authority

You have received this voluntary release authority because we have issued you with an excess concessional contributions tax assessment.

You can use it to authorise your super fund (other than a defined benefit interest fund) to withdraw funds from your super account to pay the assessed amount of excess concessional contributions tax.

You do not need to use this authority to pay the tax you owe.

Attention icon

If you do decide to send this authority to your fund, it is only valid for 90 days after the issue date on the authority and cannot be reissued after this time has expired.

If you do not send the authority to your super fund within 90 days, your fund will not be able to withdraw the amount you request.

Direction icon

When you have completed sections A, B and C, send the authority to the super fund you nominated in section B. Do not send the form to us. Your super fund will forward the form to us after they have completed sections D and E.
