Lost members statement (LMS) paper forms

The Lost members statement (LMS) is available in portable document format (PDF).

You can download the paper forms:

• Lost members statement paper form (NAT 71825, PDF, 236KB) if you are required to
• report information about lost members, or
• lodge a paper non-lodgment advice.
• Lost member form addendum (NAT 71825A, PDF, 151KB) if you have additional lost members to report.

If you are lodging for more than 100 members you must lodge the information electronically using the Lost members statement - electronic reporting specification.

Both paper and electronic LMS now include the non-lodgment advice (NLA).

You must use the only current version of the LMS form or LMS electronic specification for reporting.
