Withdrawal of superannuation guarantee

Use this form if you want to request a withdrawal of your superannuation guarantee (SG) entitlements held by us. Only full balances can be claimed and SG payments are subject to the same tax obligations as a superannuation lump sum payment. This form is available in a Portable Document Format (PDF) version.
