If you do not have a spouse and do not have children

  • Are you an Australian resident for income tax purposes?
  • Provide details of your income for Medicare levy surcharge (MLS) purposes for the relevant income year(s).
  • Did you have exempt foreign employment income? If yes:
    • What was the amount?
  • Are you covered by an appropriate level of private patient hospital cover for the relevant income year(s)? If yes:
    • Who is this policy with?
    • What excess applies to this policy?
  • Did you have private patient hospital cover for the full year? If no:
    • Please provide the dates you did have the cover.
  • Are you a prescribed person for Medicare levy surcharge purposes? If so, provide details.

If you do not have a spouse and have children

  • Are you and your children Australian residents for income tax purposes?
  • Provide details of your income for Medicare levy surcharge (MLS) purposes for the relevant income year(s).
  • Did you have exempt foreign employment income? If yes:
    • What was the amount?
  • Provide details of your children's ages and whether they are in full-time education at a school, college or university.
  • Are you and your children covered by an appropriate level of private patient hospital cover for the relevant income year(s)? If yes:
    • Who is this policy with?
    • What excess applies to this policy?
    • Did you have private patient hospital cover for the full year? If no:
    • Please provide the dates you did have the cover.

If you have a spouse and do not have children
