    We are authorised by the Taxation Administration Act 1953 to collect your tax file number (TFN). Your superannuation fund is also authorised to collect your TFN under the Superannuation Industry (Supervision) Act 1993. Your TFN will be used to identify you in our/their records. It is not an offence not to provide your TFN but if you don't there may be a delay processing this form.

    Your superannuation fund and the ATO are also authorised under the Income Tax Assessment Act 1997 (ITAA 1997) to collect the information on this form. This information will be used to administer the taxation and superannuation laws. If this information is not collected your superannuation provider would fail to meet their reporting obligations, as set out in Section 292 of the ITAA 1997 which may result in further penalties and interest charges in relation to your excess non-concessional contributions tax.

    Section 292 of the ITAA 1997 also authorises the Commissioner to give this information to one or more superannuation providers that hold a superannuation interest (other than a defined benefit interest) for you.

    Where authorised by law, we may give this information to other government departments and agencies, including:

    • Department of Education
    • Department of Employment
    • Department of Social Services
    • Department of Human Services
    • Department of Immigration and Border Protection
    • State and Federal Police
    • Australian Securities & Investment Commission
    • Australian Prudential Regulation Authority
    • Australian Bureau of Statistics
    • Reserve Bank of Australia.

    We also provide taxpayer information to treaty partners overseas under international tax agreements.
