    We are authorised by the Taxation Administration Act 1953 to request your tax file number (TFN). We will use your TFN to identify you in our records. It is not an offence not to provide your TFN however if this is not provided, there may be a delay in processing this Fairer taxation of excess concessional contributions election form.

    We are also authorised under the Income Tax Assessment Act 1936 and the Income Tax Assessment Act 1997 to collect information about individuals and super funds, either from you or from your authorised legal representative. We use this information to help us administer the taxation and superannuation laws. If this information isn’t collected we cannot action your request to release excess concessional contributions from your superannuation fund/s.

    As the person authorised to complete this form, we collect some information about you including your Tax agent number for tax agents. The information collected will allow us to contact you if we require more information on what has been provided in this form.
