    We are authorised by the Income Tax Assessment Act 1936, Income Tax Assessment Act 1997, Income Tax (Transitional Provisions) Act 1997 and the Taxation Administration Act 1953 to ask for the information on this form. We need this information to help us to administer taxation law. If the information is not collected it could increase the chance of a delay or error in your income tax assessment.

    We are authorised by the Taxation Administration Act 1953 to request your tax file number (TFN). We will use your TFN to identify you in our records. It is not an offence not to provide your TFN but if you don’t there may be delays or administrative errors in processing your application form. In addition, if you do not provide your TFN you may be unable to use the ATO’s online services to lodge this form.

    We collect some personal information about you because you signed the form. That information will allow us to check that you have the authority to complete the form and to contact you if we need more information.

    Certain information about your computer, your browser and the authentication process will also be logged by us when you authenticate online and use the ATO’s online services.

    Where authorised by law to do so, we may give certain information to other government departments and agencies, such as Services Australia, Department of Social Services and law enforcement agencies.

    We also provide taxpayer information to treaty partners overseas under international tax agreements.

    If you have an Australian Business Number, some information on this application form may be collected under the A New Tax System (Australian Business Number) Act 1999 and will be used to update your details on the Australian Business Register (ABR). Where authorised by law, selected information may be made publicly available and some may be passed to a wide range of government agencies, including Commonwealth, state and local government agencies.

    The ATO’s privacy policy contains important information about your privacy, including information about how:

    • you can access and seek correction of information we hold about you
    • to make a complaint if you think we have breached the Australian Privacy Principles
    • we will deal with any privacy complaints.
