    When to complete this application

    Complete this application if you are the legal representative of a deceased account holder who:

    • may be entitled to receive a first home saver account (FHSA) government contribution, and
    • requires a government contribution entitlement be paid early.
    Attention

    A legal representative is an executor or administrator of an estate of a deceased person.

    End of attention

    Information you must provide

    You must provide the following information if you have not already done so:

    • a certified copy of the account holder's death certificate
    • a final income tax return for the deceased account holder - if you are not required to lodge a final income tax return, you must lodge a FHSA notification of eligibility (218KB, NAT 72947)
    • a letter from the account holder's first home saver account provider (on letterhead) detailing
      • personal contributions made in the financial year of the account holder's death
      • the closing balance
      • the first home saver account number, and
      • the provider's Australian business number (ABN)
       
    Attention

    The first home saver account must be closed before you request this information.

    End of attention
    • proof of the account holder's change of name, if applicable
    • proof of your change of name, if applicable
    • proof that you are a legal representative of the account holder.
