What you can do with this form

This form is for Australian Defence Force veterans. You can use this form to request a review of your income assessments due to changes in components of your payments made by the:

• Commonwealth Superannuation Corporation (CSC)
• Department of Veterans Affairs (DVA).

The form can also be completed and lodged by someone else on the veteran's behalf.

What you will need

To complete the form, you will need to provide:

• your full name
• your tax file number (TFN)
• details of who we should contact about the review (the veteran or the person completing and lodging the form on their behalf)
• the documents CSC or DVA provide to you, such as your Disability Superannuation Benefit (DSB) Determination letter or your DVA Overpayment letter.
