What you can do with this form

This form is for Australian Defence Force veterans. You can use this form to request a review of your income assessments due to changes in components of your payments made by the:

  • Commonwealth Superannuation Corporation (CSC)
  • Department of Veterans Affairs (DVA).

The form can also be completed and lodged by someone else on the veteran's behalf.

Who should consider lodging this form?

The compensation system applying to veterans can be complex. Tax may be overpaid.

This form is particularly useful if:

  • you’ve received backdated or lump sum payments from CSC
  • you've received a backdated Disability Superannuation Benefit (DSB) Determination from CSC
  • you’ve had to repay DVA or Centrelink from an arrears
  • you believe your income assessment may not reflect your actual taxable income due to these changes.

What you will need

To complete the form, you will need to provide:

  • your full name
  • your tax file number (TFN)
  • details of who we should contact about the review (the veteran or the person completing and lodging the form on their behalf)
  • the documents CSC or DVA provide to you, such as your DSB Determination letter or your DVA Overpayment letter.
