Who should use this form
Law enforcement and other government agencies can use the Information disclosure request form (NAT 75124) to request protected taxation information from the Australian Taxation Office (ATO). The request is pursuant to section 355-65 and section 355-70 of Schedule 1 to the Taxation Administration Act 1953 (TAA).
Instructions
It is important to follow the instructions outlined in the portable document format (PDF) application form.
Failure to do so could result in your request being rejected by ATO systems.
To complete the form:
- Download the PDF form below – you can only do this on a desktop or laptop device.
- Right-click on the link and select ‘Save (link) target as’ (wording may vary depending on your internet browser).
- Open the form using Adobe Acrobat Reader/Pro and enable JavaScript if prompted.
- Complete the form on your computer.
- Lodge your form.
