We will use this information to determine your eligibility for early release of superannuation on compassionate grounds. Where authorised by law, we may also collect certain information indirectly, from third parties, such as other government departments and agencies, financial institutions and employers.

We are authorised by the Taxation Administration Act 1953 to request your tax file number (TFN). We will use your TFN to identify you in our records. We may disclose the information collected on this form to your superannuation fund. Where authorised by law to do so, we may also give this information to other government departments and agencies, such as Services Australia, Department of Social Services and law enforcement agencies.

We also provide taxpayer information to treaty partners overseas under international tax agreements.

If you have an Australian Business Number, some information on this application form may be collected under the A New Tax System (Australian Business Number) Act 1999 and may be used to update your details on the Australian Business Register (ABR).
