We are authorised by the Taxation Administration Act 1953 to request your tax file number (TFN). We will use the TFN to identify you in our records. It is not an offence not to provide your TFN. However, if this is not provided, there may be a delay in processing the form.
We are also authorised under the Taxation Administration Act 1953 and the Superannuation Guarantee (Administration) Act 1992 to ask for the information on the Super guarantee opt out for high-income earners with multiple employers form.
We use the information you provide to help us administer the taxation and superannuation laws and to decide whether to issue an employer shortfall exemption certificate. If we do not collect this information, we may be unable to check our records or make a decision, which could delay or affect the processing of your application.
By law, we are authorised to give the information on this form to the employer, or their authorised agents, for the purpose of issuing a shortfall exemption certificate.
Where authorised by law, we may give this information to other government departments and agencies including law enforcement agencies. We may also provide taxpayer information to treaty partners overseas under international tax agreements.
