Section D: Declaration and signature – CIVs

How to complete Section D: Declaration and signature in the RTP schedule for collective investment vehicles.

Total number of pages being lodged

Write the total number of pages you're lodging, including any attachments, in Total number of pages being lodged.

Declaration

When the RTP schedule is complete, the public officer or the trustee (as the case requires) must make a declaration that the information in the schedule and any attachments is true and correct.

To make the declaration select the check box at I declare that the information on this schedule is true and correct.

In the declaration, include the:

  • public officer's or trustee's name and daytime phone number
  • public officer's or trustee's written or digital signature
  • declaration date. In the PDF version, write the date in the box next to Date.

Signing the declaration

If you're lodging by post the public officer or trustee must print and sign the schedule.

If you're lodging through Online services for business or Online services for agents, the public officer or trustee must check the I declare that the information on this schedule is true and correct box. The public officer or trustee doesn't need to sign a print copy and should lodge the electronic form (rather than scanning a version) through our online services.
