) the private health insurer that issued the policy in respect of which a person is a participant if the person no longer wishes to be registered in respect of the policy.

approved form and no more than 14 days (or any other period determined by the Chief Executive Medicare) after receiving the notice.
23-30(2A)
A notice given by the private health insurer under subsection
(2)
must:
(a)
be given no more than 7 business days (or any other period determined by the Chief Executive Medicare under subsection
(4)
) after the withdrawal notice is received by the private health insurer; and
(b)
be given electronically in accordance with the electronic system approved by the Chief Executive Medicare under section
333-16
for the purposes of this paragraph; and
(c)
include the information required by that system to be included in the notice; and
(d)
be accompanied by any documents required by that system to accompany the notice.

23-30(3)
If notified under subsection
(2)
